Griseofulvin

Griseofulvin is an antifungal medication that belongs to the class of drugs known as griseofulvins. It inhibits fungal cell division by disrupting the mitotic spindle structure, preventing the formation of functional mitotic spindles. This action leads to the inhibition of fungal cell mitosis, ultimately impairing fungal growth and replication.

Griseofulvin is indicated for the treatment of the following fungal infections:

  • Tinea corporis (ringworm of the body)
  • Tinea pedis (athlete's foot)
  • Tinea cruris (ringworm of the groin and thigh)
  • Tinea capitis (ringworm of the scalp and hair)
  • Tinea barbae (ringworm of the beard)
  • Tinea unguium (onychomycosis, fungal infection of the nails)

For adults, a daily dose of 375 mg (administered as a single dose or divided doses) is generally effective for most patients with tinea corporis, tinea cruris, and tinea capitis. In cases of more resistant fungal infections, such as tinea pedis and tinea unguium, a recommended divided dose is 750 mg.

In pediatric use, an effective dose of ultramicrosize griseofulvin is approximately 7.3 mg per kg of body weight per day. Based on this, the following dosage schedule is suggested:

  • For children weighing 16-27 kg: 125 mg to 187.5 mg daily.
  • For children weighing over 27 kg: 187.5 mg to 375 mg daily.

Common adverse reactions associated with the use of Griseofulvin include:

  • Headache
  • Nausea
  • Vomiting
  • Diarrhea
  • Dizziness
  • Fatigue
  • Photosensitivity
  • Rash

Physicians should be aware of the following warnings and precautions when prescribing Griseofulvin:

  • Hepatotoxicity: Griseofulvin may cause hepatotoxicity, and periodic monitoring of liver function is recommended, especially in patients with pre-existing liver disease.
  • Pregnancy Category X: Griseofulvin is contraindicated in pregnant women due to the risk of fetal abnormalities. Adequate contraception should be ensured during and for one month after discontinuation of therapy.
  • Photosensitivity: Patients should be advised to avoid excessive sunlight exposure and to use sunscreens during Griseofulvin therapy.
  • Drug Interactions: Griseofulvin may interact with several medications, including oral contraceptives, leading to decreased contraceptive efficacy. Additional contraception methods should be considered during Griseofulvin treatment.

Contraindication

Griseofulvin is contraindicated in the following situations:

  • Hypersensitivity to Griseofulvin or any component of the formulation.

This drug is contraindicated in patients with porphyria or hepatocellular failure.
